How to Start the Vial Quest
First, head to the small house with the Nurse NPC and accept her quest. She will ask you to collect 10 vials spread across the world. Before you begin, it helps to bring mobility tools like a gravity coil or a fast boat. Many vials are placed high on cliffs or structures that require climbing.
Vial 1: Scoria Reach Volcano
Travel to Scoria Reach, the new volcanic island behind Sunstone. Climb to the top of the large volcano. The vial is located near the summit. This climb is one of the hardest, so mobility gear makes it much easier.
Vial 2: Terrapin Island Near Olm Cave
Go to Terrapin Island near Moosewood and enter the Olm cave. Head to the left side inside the cave to find the second vial.
Vial 3: Grand Reef Pirate Ship
Sail to Grand Reef where the pirate ship is docked. Jump onto the ship and move toward the wheel area at the front/back deck section. The vial is placed on the ship.
Vial 4: Mushgrove Back Area
Head behind Moosewood to Mushgrove. Stay on your boat and travel to the very back of the island. The vial is placed near the rear shoreline.
Vial 5: Forsaken Shores
Travel to Forsaken Shores and climb upward the mountain using ladders and parkour. The vial is hidden behind a rock near the upper section of the island.
Vial 6: Castaway Cliffs Docks
Go to Castaway Cliffs and move near the docks. Find the pirate ship and climb above it. The vial is positioned above the ship structure.
Vial 7: Lost Jungle Summit
Sail to Lost Jungle and climb to the top of the island. The vial is located near one of the newer fishing rod spots, tucked under a mushroom.
Vial 8: Roslit Potholes
Travel to Roslit and look for the small pothole formations. Jump across them to reach the vial hidden in that rocky section.
Vial 9: Ancient Isles Crane
Go to Ancient Isles and carefully navigate past the sea traps. Climb to the highest point of the island where a crane is located. The vial sits above the crane.
Vial 10: Snowcap Hidden Path
The final vial is at Snowcap. Take the hidden leafy path near the old Lost Rod location and follow it to the end. The vial is tucked away at the far edge.
Completing the Quest and Getting Cerebra
After collecting all 10 vials, return to the Nurse NPC. She will give you a short follow-up quest requiring additional items. Complete this final step and talk to her again to receive the Cerebra rod. Cerebra stands out for its visual design and unique animation effects.
