1. Bonnie the Serial Killer
Bonnie is a special character who claims to have a suitcase. Depending on your choices, two endings can occur.
Turn Bonnie Over to the Police
If you report him to the duck police officer, Bonnie escapes and knocks the officer unconscious.
Hide Bonnie
If you decide to protect him, he’ll thank you and reward you with a second coffee machine. Interestingly, Bonnie uses the scalpel he requests to threaten players. You can reject his request by closing the shutters when the timer expires.
2. Bed Monster
The Bed Monster appears underneath patient beds inside a transparent red area. Stepping into the red zone causes the creature to grab you and heavily damage your sanity. To remove it:
- Hold Maple Syrup.
- Approach the red area.
- The monster will disappear.
The Bed Monster can also attack patients and other anomalies.
3. Stalker
This creature appears as a long black rabbit with an unsettling smile. Looking directly at it causes sanity damage. It can spawn almost anywhere, including around corners. To survive:
- Avoid staring at it.
- Keep moving past it.

4. Don’t Look Up
This anomaly appears as a giant mass of eyes and flesh attached to ceilings in patient rooms or the CCTV room. Looking upward causes it to grow larger and repeatedly drain your sanity. The solution is simple:
- Keep your camera aimed downward.
- Never stare at the ceiling.
5. Void Creatures
These black creatures usually appear in groups throughout the hospital. If touched, they will:
- Chase you.
- Drain sanity.
- Steal your items.
Simply run away and they will eventually disappear into black smoke.
6. Skinwalker
Skinwalkers are among the most dangerous enemies. They appear because an anomaly was accidentally allowed through check-in. Their appearance often includes:
- Sharp teeth.
- Large mouths.
- Distorted faces.
Skinwalkers attack both players and patients.
If They’re Attacking a Patient
Approach them from behind and attack.
If They Grab You
Press E repeatedly to hit pressure points and escape.
Ignoring them causes rapid sanity loss.
7. Head Banger
The Head Banger appears at the second check-in window with hollow eyes while repeatedly smashing its head. The safest solution is giving it coffee. Trying to stop it without coffee causes severe sanity damage.
8. Surgery Tentacles
Large purple tentacles can spawn inside Room 8. Once they appear, surgery must be completed before the 45-second timer expires. Failing to do so kills the patient. Important:
- Never shoot them.
- Never use a taser.
Doing so instantly kills the patient.

- Three Red Eyes Patients with glowing red eyes are always anomalies.
