Completing the Journal in Animal Hospital requires much more than treating patients. Throughout your shifts, you’ll encounter mysterious anomalies, unique NPCs, hidden events, and dozens of achievements that gradually fill your Journal. While some entries unlock naturally through gameplay, others require completing specific tasks or interacting with rare encounters. This guide explains how to unlock every Journal category, including anomalies, NPCs, secret endings, and achievements.


How the Journal Works in Animal Hospital

The Journal tracks nearly every major discovery you make during your time at the hospital. As you continue playing, you’ll unlock entries by:

  • Encountering anomalies
  • Meeting important NPCs
  • Completing special objectives
  • Reaching progression milestones
  • Discovering hidden interactions

Finishing every section takes time since several events only appear under certain conditions.


All Anomaly Journal Entries

Anomalies are one of the largest categories in the Journal. Each one has its own unlock requirement.

Massive Eyes

To encounter the Massive Eyes anomaly:

  • Ignore the “Don’t Look Up” warning.
  • Look directly at the giant eyes on the ceiling.
  • Continue staring until the encounter begins.

Doing this unlocks the Massive Eyes Journal entry.


Ghost

Ghosts appear after patients die. To unlock this entry:

  • Wait for a ghost to spawn from a deceased patient.
  • Allow the ghost to reduce your Sanity.

After being affected, the Ghost entry is added to your Journal.


Stalker

The Stalker is one of the simpler anomalies to discover.

Simply look directly at the Stalker when it appears to register the encounter.


Mimic

To unlock the Mimic entry, you must survive one of its attacks.

You can do this by:

  • Escaping the Mimic yourself.
  • Saving another player during the attack.

Successfully surviving the encounter records the Mimic in your Journal.


Hider

Allow a Hider to catch your character.

Once captured, the anomaly is successfully registered.


Death Ritual

The Death Ritual event involves several lit candles.

To complete this encounter:

  • Wait for the Death Ritual event.
  • Use Eye Drops on the burning candles.
  • Extinguish them before the ritual finishes.

Headbanger

If you encounter a Headbanger, interact with it using Coffee.

Giving Coffee to the creature unlocks its Journal entry.


Camera Monster

The Camera Monster only appears under specific circumstances.

To improve your chances:

  • Avoid repairing every broken security camera.
  • Continue monitoring the remaining cameras.

Eventually, the Camera Monster may appear, allowing you to record the encounter.


Bed Monster

There are two ways to complete this entry:

  • Feed the Bed Monster Maple Syrup.
  • Allow it to capture your character.

Either method counts toward your Journal.


Tendril

The Tendril event occurs during surgeries.

Watch for:

  • Surgery in Room 8.
  • Tentacles growing from the patient’s body.

Witnessing the event unlocks the Tendril entry.


Slime Bot

Allow the Slime Bot to swallow your character.

Once this happens, the anomaly is recorded in your Journal.


All NPC Journal Entries

Meeting important characters also contributes toward Journal completion.

Barney

There are three Barney-related entries.

Barney First Encounter

Meet Barney for the first time.


Barney Ending A

Choose to hand Barney over to Officer Duckman.

This unlocks one ending along with its Journal entry.


Barney Ending B

Instead of turning Barney in, hide him from Officer Duckman.

Choosing this route unlocks the alternate ending.


Rafu

Rafu has several Journal entries.

First Encounter

Simply meet Rafu for the first time.


Final Encounter

Continue interacting with Rafu throughout multiple shifts until reaching the final conversation.


Secret Ending

The Secret Ending requires several steps.

First:

  • Continue speaking with Rafu across multiple encounters.
  • Eventually, he’ll give you a special key.

Next:

  • Visit Room 5.
  • Tear down the poster on the wall.
  • Reveal the hidden safe.
  • Use Rafu’s key to open it.

Doing so unlocks Rafu’s Secret Ending entry.


Ron

Ron also has multiple Journal entries.

First Encounter

Meet Ron for the first time.


Final Encounter

Continue interacting with Ron until reaching his final dialogue.


Liz

Liz’s storyline requires taking photographs.

First Encounter

When meeting Liz:

  • Listen carefully to the names she provides.
  • Use her camera.
  • Photograph the requested anomalies.

Final Encounter

Continue helping Liz collect evidence.

Eventually she’ll thank you, completing her storyline and unlocking the final Journal entry.


Sam

Simply meeting Sam for the first time records the encounter.


Dr. Harlow

Speak with Dr. Harlow whenever you encounter them.

Doing so unlocks another NPC Journal entry.


Shift Progression Achievements

Several achievements are unlocked simply by continuing to play.

Reach the following shifts:

  • Shift 10
  • Shift 25
  • Shift 50
  • Shift 100

Each milestone awards its own achievement.


Patient Treatment Achievements

Treating patients is another major source of Journal progress.

Continue helping patients until you’ve treated:

  • 500 total patients

Additional treatment milestones may unlock along the way.


Secret Achievements

Several hidden achievements require unusual actions.

No One Will Know

Wait until a patient faints.

Instead of helping them, place the unconscious patient into the trash.


Friendly Fire

Acquire either:

  • A Gun
  • A Taser

Use the weapon on an innocent patient to unlock the achievement.


Maple Trick

Give Maple Syrup to the Bed Monster.

This unlocks the Maple Trick achievement.


Ritual Trick

During a Death Ritual:

  • Equip Eye Drops.
  • Use them on the ritual candles.

Successfully extinguishing them awards the achievement.


Eye Drops

When the Massive Eyes anomaly appears:

  • Use Eye Drops on the eyes.

Doing this unlocks another hidden achievement.


Lifesaver

Purchase a Teddy Bear from the Staff Shop.

Use it to revive another player.

Successfully reviving someone unlocks Lifesaver.


Camera Fixer

Whenever a surveillance camera breaks:

  • Repair it.

Repairing your first camera awards the Camera Fixer achievement.


Trash Collector

Dispose of at least:

  • 15 items

Once you’ve thrown away fifteen objects, the achievement unlocks.


Rejector

Close the office shutters to reject an incoming anomaly. Many players unlock this naturally during normal gameplay.


Bed Monster Attacks Hider

This achievement requires two anomalies at once. To complete it:

  • Find a Hider.
  • Lure it toward a Bed Monster.
  • Allow the Bed Monster to attack the Hider.

This is considered one of the more difficult achievements because it depends on multiple random events.


Bed Monster Attacks Mimic

Similar to the previous achievement:

  • Find both a Mimic and a Bed Monster.
  • Lead the Mimic toward the Bed Monster.

If the interaction occurs successfully, the achievement unlocks.


Coffee for the Visitor

Give Coffee to a Headbanger. Although this interaction also records the anomaly, it additionally unlocks its own achievement.


Wrong Treatment, Right Target

Use the incorrect treatment item on a patient. If that patient turns out to be an anomaly instead of a normal patient, you’ll receive this hidden achievement.


Ghost Hunter

Equip the Scanner. Use it to scan a Ghost during an encounter. Successfully scanning the ghost unlocks Ghost Hunter.


Tiny Anomaly

Allow one of the miniature anomalies to enter the hospital. Instead of rejecting it, check it in like a normal patient. Doing so unlocks the Tiny Anomaly achievement.


Completing the Journal in Animal Hospital requires patience, exploration, and a willingness to experiment with the game’s many hidden mechanics.
